Here's how to get started:

1. Log in to your Mixform account.
2. Click the Referrals Tab at the top of the page to get your affiliate code.
3. Include your affiliate code in your e-mails when recommending Mixform to friends.
4. Each time someone signs up for a Mixform account using your affiliate code you'll immediately start adding up 25% of his or her monthly fees.
5. You can request one check every month to receive your affiliate earnings. The check must be between $100 and $500 dollars.
6. Your affiliate earnings will not expire, they carryover the next month, as long as your Mixform account is active.
